This policy describes information handled by this NDB website. It does not replace a clinical privacy notice or the privacy terms of an external appointment service.
Information you provide
Callback forms collect your name, email, optional phone number, preferred location and permission to respond. Account features collect account credentials and email. Commerce records may include cart contents, orders, payment references and fulfillment details. Scheduling records include your chosen service, provider, location, appointment time, contact details, policy acknowledgment and payment status. They are used for appointment administration, not marketing. Please do not submit medical information through these forms.
How information is used
Information is used to respond to requests, maintain accounts, manage eligible purchases and protect the service. Website contact details are not automatically enrolled in marketing. No advertising trackers or marketing analytics are loaded on this website.
Cookies and security
Essential cookies support authentication, cart sessions and protection against forged requests. Access to administrative records is restricted. Limited security records help control abuse. Medical intake, diagnoses and laboratory results are not collected here.
External services
Links to Square booking, maps and social services take you to separately operated websites with their own privacy policies. When a hosted payment service is used for an eligible purchase, card details are handled by that service rather than entered into this website.
Retention and requests
Contact information is retained to handle the request and related business needs. Order information may need to be retained for applicable recordkeeping. Contact NDB to request access, correction or deletion; some records may need to be retained where required.
